The expression "
(every) once in a while" means occasionally, and is located between "sometimes" and "never" in terms of frequency:
100% always
often
50% sometimes
occasionally //
once in a while0% never
Some examples:
Once in a while I meet my friends in the park and play frisbee.
Even though Tokyo is crowded, I like to go there
every once in a while.
Using "every" with "once in a while" is optional, and does not change the nuance or meaning. But in my opinion, it's usually more natural to include "every."
